Many people find aftercare challenging. As such, when a Quora query about caring for stage 3 kidney disease popped up in our feed — “Is it possible for stage 3 kidney disease to remain stable for a long time, and what lifestyle changes can help achieve that?” — we reached out to Dr Nikhil Bhasin, consultant nephrologist and renal transplant physician, Wockhardt Hospitals, Mumbai Central, who said that stage 3 chronic kidney disease (CKD) does not automatically lead to kidney failure.

“Many patients live for years, sometimes decades, without their condition getting worse, provided it is detected early and managed well. Stability depends on controlling the factors that further damage the kidneys, such as high blood pressure, diabetes, and recurrent infections. With regular check-ups and a disciplined lifestyle, kidney function can stay steady,” explained Dr Bhasin.

Stressing that the most common triggers are poorly controlled diabetes and high blood pressure, the expert told indianexpress.com: “Uncontrolled blood sugar and high blood pressure can scar the kidneys over time. Frequent use of painkillers, smoking, dehydration, and ignoring early warning signs like swelling or fatigue also speed up the damage. Many patients feel ‘normal’ at stage 3 and stop taking precautions, that’s when problems start.”

What lifestyle changes make the biggest difference?

Small, consistent changes can have a greater impact than drastic ones. Dr Bhasin said reducing salt intake helps control blood pressure and prevents fluid retention. “A balanced diet with moderate protein, instead of high-protein fad diets, is important. Staying well hydrated, quitting smoking, limiting alcohol, and maintaining a healthy weight help reduce stress on the kidneys. Regular physical activity, like walking, also improves blood sugar and blood pressure control,” said Dr Bhasin.

Do patients need to stop eating protein altogether?

Dr Bhasin stressed that protein is important, but too much can be harmful. “The key is moderation and choosing high-quality sources. Diet plans should always be personalised, ideally with guidance from a nephrologist or renal dietitian, rather than following generic advice online,” said Dr Bhasin.

It’s important to monitor at stage 3 regularly. “Periodic blood and urine tests help track kidney function and catch complications early. Adjusting treatment at the right time can significantly delay progression. Consider stage 3 as a window of opportunity. This phase is where timely action can have the greatest effect.”

What to note? 

Dr Bhasin affirmed that stage 3 kidney disease is “not a dead end.” “With awareness, changes in lifestyle, and medical supervision, many people can continue to lead active, productive lives without ever needing dialysis.”
